At EmpowHer Women’s Health & Wellness, we care about our patients and we schedule our appointments so that each patient receives the right amount of time to be seen by our physicians, nurse practitioners, and staff. This is why it is very important to us that you arrive on time and keep your scheduled appointment with us. As a courtesy, we will send out a reminder text about your appointment in advance of your appointment time.
If your schedule changes and you are unable to keep your appointment, please contact us as soon as possible so that we may reschedule you and accommodate patients who are waiting for an appointment. As a courtesy to our office and to those who are waiting to be scheduled, please give us at least 24 hours advance notice.
If you do not cancel or reschedule your appointment with at least 24 hours notice, we will apply a $35 “no show” service charge to your account. This amount is not reimbursable by your insurance company, and you will be billed directly.
After 3 consecutive no-shows to your appointment, we reserve the right to terminate our relationship with you as a patient. Additionally, we understand that delays can happen. However, we do need advance notice if you are running behind to see if we can still accommodate you. Please notify us of your delay. For new patients, you must be present 15 minutes prior to your scheduled appointment time or we will need to reschedule you. For established patients, if you arrive 15 minutes past your appointment time, you will be rescheduled.
Your signature below indicates that you understand our no show, cancellation, and late policies and that you agree to provide a credit card number which will be charged $35 for any no-show or late cancellation of a scheduled appointment. If you cancel or reschedule with at least 24 hours notice then you will not be charged
